Online Renewal is available from June 21, 2010 to August 31, 2010. Please click on the link provided to begin your online renewal process. If you are not allowed to renew online, please contact the Board Office at (602) 274-0236. Some licensees and certificate holders must complete additional documentation before completing the renewal process.
Arizona Licensed Physical Therapists and Certified Physical Therapist Assistants must renew before September 1, 2010. Failure to timely renew will cause your license or certificate to be administratively suspended until the license or certificate is reinstated or expires after three (3) years of administrative suspension. Practicing or working, as a physical therapist or physical therapist assistant, without an active license or certificate, regardless of the period of time, may result in disciplinary action.

Physical Therapy Services-Business Entity Registration
On April 23, 2010, Governor Brewer signed HB 2123 which creates a requirement for some business entities that provide physical therapy services in Arizona to register with the Board of Physical Therapy beginning in August 2011. The law will be effective July 29, 2010. Please contact Board staff with any questions regarding these changes in law.

Arizona State Board of Physical Therapy
On October 6, 2009, the Governor’s Regulatory Review Council approved changes to Board of Physical Therapy Arizona Administrative Code (Rules) Title 4. Professions and Occupations Chapter 24, Articles 1, 2 and 3 which will take effect on December 7, 2009. The changes include an updated incorporation of reference for the course evaluation tool used to review foreign educated physical therapist applicants’ education, adding a time frame for processing license and certificate renewals to the Board’s Time-frames table, and changes to the Lawful Practice rules. The Board encourages all licensees and certificate holders to review the new rules as they will take affect in a short period of time. Follow the link above to view the updated rules.
